Ichigo's mother has never been brought up much and yet Aizen felt it was important to bring up his father and his mother during their initial battle, and note he mentioned she was human, not shinigami. Isshin and Masaki probably met somewhere on the battlefield. When Isshin and Masaki got together, they gave up their roles as a shinigami and Quincy to live as humans.
Everyone was happy, until the Grand Fisher hollow felt Masaki's Reiatsu. It lured her into its trap by getting Ichigo. Without her Quincy cross she is helpless and dies.
