The Elderly Services Program ESP provides in-home care for eligible older adults in Clinton County who might otherwise be forced to leave their homes and enter a nursing facility. Annually, more than county older adults receive care in their homes through ESP. Clinton County ESP provides services such as housekeeping and personal care, nutritious meals, emergency response systems, transportation, caregiver support and more.
ESP clients also have a care manager who advocates for their needs, answers questions, and makes sure they receive the right services. The Elderly Services Program is made possible thanks to the generous support of Clinton County taxpayers via a senior services tax levy. Other funding comes from federal and state money, grants, donations and client co-payments. To serve as many people as possible with tax dollars available, some clients are required to help pay for the cost of their care through a co-payment.
Co-payments are set on a sliding scale depending on income and medical expenses. There is no co-payment for FastTrack Home services or home-delivered meals, but donations are welcomed to help provide services to more older adults. COA handles assessment, intake, and care management.
COA also processes all billing, and provides program planning, reporting, auditing, contracting and provider services, technical assistance and community engagement.
Agencies - many of which are based in Clinton County - contract with COA to provide in-home services such as housekeeping help, personal care and medical transportation.
